All certifications expire the first day
of January the year after they are issued. Certification obtained after
September 1st will not expire until the year following the year
of issue.
Example 1 A Basic Certification obtained on August 31, 2003
would expire on January 1, 2004
Example 2 A Basic Certification obtained on
September 1, 2003 would expire January 1, 2005.)
A copy of the letter from the Texas Department of Health
indicating that you have passed the TDH ACO Certification Course or a copy
of the TDH Certificate of Completion.
A copy of the letter from STACCA indicating that you have
passed the Instructor or Certified Euthanasia Training Course or a STACCA
Certificate of Completion.
A copy of the letter from a recognized training institution
indicating that your have passed their Certified Euthanasia Training Course
or their Instructor Training Course or a Certificate of Completion issued by
that institution.
A copy of a current certification from another certifying
agency indicating the level of proficiency for which you are qualified.
